This scar is likely a manifestation of a microform cleft, the mildest form of cleft lip, appearing as a scar or a small notch. Roughly 6.8 thousand babies in the United States are born each year with some form of cleft lip or cleft palate. Phoenix has mentioned in interviews that he believes the scar to be a birthmark resulting from an "act of God."
